Unable to install/enable DAC+ Pro

Hi,

I am trying to set this up for the first time & have reached a couple of dead ends. Hopefully I have missed something simple?

I am trying to install a DAC+ Pro to a PI 3B+
I first tried HiFiBerryOS, but it failed to load, so I tried it on a spare PI 3B+. I got the same error with both so I do not believe it is a hardware fault:
[Firmware Bug]: Bootloader left irq enabled: bank 1 irq 9.
This was early in the boot process. I have found no way to interrupt it & it halts the boot process, so I was unable to do anything with it.

I have tried enabling it on 2 installations of Raspberry OS, both v10/Buster. Firstly 1 which was already installed about 9 months ago then updated prior to adding the DAC. I then downloaded the latest available (July 2020), but having followed the guide, both had the same issues.

Remove dtparam=audio=on from /boot/config.txt --file did not exist

Add dtoverlay=hifiberry-dacplus to /boot/config.txt --created file & added this line
permissions on this file are -rwxr-xr-x 1 root root.

Rebooted.
Added Alsa as described in the instructions
Rebooted again.

aplay -l gives the following when logged in as root:
**** List of PLAYBACK Hardware Devices ****
card 0: b1 [bcm2835 HDMI 1], device 0: bcm2835 HDMI 1 [bcm2835 HDMI 1]
Subdevices: 4/4
Subdevice #0: subdevice #0
Subdevice #1: subdevice #1
Subdevice #2: subdevice #2
Subdevice #3: subdevice #3
card 1: Headphones [bcm2835 Headphones], device 0: bcm2835 Headphones [bcm2835 Headphones]
Subdevices: 4/4
Subdevice #0: subdevice #0
Subdevice #1: subdevice #1
Subdevice #2: subdevice #2
Subdevice #3: subdevice #3

I have tried connecting it to an amplifier, but was not surprised that I got no audio.
