Does anyone know of a fix?

Too many unknown variables - which audio board, which software, which version etc.

Volumio Linux volumio 4.1.18-v7+
Hard drive is a wd model -bus powered but connected to a powered hub
Mp3 files have no issues
Flac files drop very often - too often to be usable
I've read that it maybe an oversampling problem.
The audio doesn't seem to be playing at the right speed along with the cutting out of sound.
Just giving this a bump.
Is anyone able to offer any assistance?

this sounds indeed like a buffer problem. Difficult to tell you an optimal buffer size, since it depends on the transmission speed of the USB harddrive, the data (HD flac need of cause more then a ripped CD in flac), if some resampling is done etc.
Just try to increase the buffer size, which should hopefully solve the problem.

I've tried 2048. 4096 and 8192 but that didn't seem to make a difference.
Can I go higher than that?
They hard disk does state it goes to USB 3.0

If the buffer size doesn't fix the issue, what else can I do?
Not just the system but storage?
P.s. Just noticed hub thing is only USB 2. Could that be causing an issue?
The Raspberry Pi itself uses only USB 2.0, so a different hub would not change this. What kind of flac are you using (a ripped CD with 44.1 kHz/16 bit, or some HD files)? With increasing the buffer, you should at least hear the dropouts less often, if they are caused by this...

It's just not the dropping out of audio, the bits that do play sound like they are playing back a little too fast if that makes sense.
What's the max size of buffer? I have a 4GB card.
OK just to update incase other people get similar issues.
Tried one of the files from ramplay which was also guff.
Thinking it might be the file. It was a full album file.
Tried a few other files I made and initial playback seemed ok
Tried the same file on a Windows machine with vlc and the file plays back without issue

I have the same problem. Running flac files stutters. Config: DIGI plus pro, PI 3b, wirles usb. Flac 44.1/16, feeding vdac. source: qnap via smb fileshare via wifi. Any advice? Squeezebox touch works perfectly
Btw, runing kodi (libreelec) on the pi
WiFi can be problematic. Try using Ethernet.
BTW: If you're also using a Squeezebox, why don't you use PiCorePlayer or Max2Play to connect to your LMS?

I don't play anything over USB anymore. I do t get any stutter over wifi
Don't want to use lms, trying for aternative now. will try with qnap mediacenter or twonkymedia. And wifi should not be the problem but to eliminate will try cabled.
I've been going through a stutter with hd flac (Volumio, RBP3, NAS, and WiFi) I upgraded the power supply to the suggested 5v 2.5A and its sorted the issue.
